Welcome to our practice! Please read the following terms and conditions carefully, as they highlight key elements of our clinical and financial policies for the purpose of scheduling an appointment. Prior to your first appointment, you will receive and complete intake paperwork where this information is provided in greater detail. Clinical Policy
Consent For Treatment
Clients are responsible for reading intake paperwork carefully and noting any questions for the clinician. This paperwork asks for some of your information and explains what you can expect at your first appointment. Once you sign the official agreement, you (and your legal guardian, if it applies) consent to the use of your personal health information for purposes of treatment, payment, or health care planning.
Medical Referrals
Your medical provider and clinician may need to work together at times; for example, if you are taking medications. Your clinician will get written permission from you (and your legal guardian, if it applies) in advance to share information with your provider. The only time information will be shared, even without your permission, is if you are doing something that puts you at risk for serious and immediate physical harm.
Financial Policy
Copays for clients covered by insurance are due at the time services are rendered. For clients who are not using insurance, or are using an insurance plan with which their clinician is not contracted, payment in full is due at the time of service. Upon request, we will provide you with a Visit Summary to file with your insurance company for reimbursement.
